San Francisco Campus for Jewish Living :

Founded in 1871 as the Hebrew Home for Aged and Disabled and previously referred to as the Jewish Home, the San Francisco Campus for Jewish Living (SFCJL) has a long and storied history of providing high-quality care to the communitys most vulnerable older adults.

Guided by the values and principles of the Jewish tradition, our mission is to enrich the lives of older adults from all faiths and backgrounds.

Situated in the citys Excelsior neighborhood on a state-of-the-art, nine-acre campus, SFCJL includes the 191-unit Frank Residences assisted living and memory care (open since 2020), and the 375-bed Jewish Home and Rehabilitation Center (JHRC), which has the distinction of being the largest, private, nonprofit distinct-part skilled nursing facility in the state of California.

Additionally, our campus includes a 12 bed, short-term acute psychiatric unit, unique for its specialization in mental healthcare for older adults.

All combined, SFCJL serves a vibrant and diverse population of more than 2,500 patients and residents annually.

The need for our services is only expected to increase. According to US Census Bureau statistics, the American population 65 and older is expected to swell from 56 million in 2020 to 81 million by 2040.

Position Overview :

Reporting to the SFCJL President and Chief Executive Officer, the CMO is the most senior physician at the San Francisco Campus for Jewish Living.

The CMO is an exceptional clinician and physician leader with a passion for the care of older adults. In collaboration with a mission driven senior executive team, the CMO contributes to all strategic and operational planning at SFCJL that involve clinical programs and will be an instrumental leader in rethinking care models and provider staffing models for the future of skilled nursing and senior care at SFCJL.

The CMO has responsibility for the strategic oversight and management of the clinical practice of all physicians and licensed providers practicing in SFCJLs complex skilled nursing and senior living environment.

In partnership with the Director of Nursing, the Licensed Nursing Home Administrator and the Executive Director of Frank Residences, the CMO assures exceptional quality, safety and experience of care for SFCJL patients and residents and assures that care provided is in accordance with all local, state, and federal regulations.

The CMO is responsible for standards, coordination, surveillance, and planning for improvement of medical care at SFCJL. The CMO serves as the President of the SFCJL Medical Staff, provides medical directorship for the short-term skilled nursing program and the Frank Residences, and oversees the campuss other medical directors.

The CMO is a member of the Senior Executive Leadership Team and represents medical staff interests and clinical priorities to the Executive Team and the Board of Trustees.

The CMO is an exceptional clinician who maintains, at minimum, a small clinical practice at SFCJL.

Essential Functions :

Medical Directorships. The CMO holds the medical directorship for the following :

Medical Director Short Term Skilled Nursing and Rehabilitation

Medical Director Frank Residences- New Medical Directorship

Attends weekly rounds with Frank clinical team, provides guidance related to complex admissions and when residents primary providers are not readily available

Collaborates with Frank leadership team on clinical program development

Medical Director Oversight-

Provides direct oversight of SFCJL medical directors. Sets expectations for performance and accountability, provides mentorship and assures professional development.

Medical Staff and Physician / Advanced Practice Provider (provider) Oversight

Oversees all medical staff functions including appointment, re-appointment and onboarding

Assures SFCJL medical staff functions operate in compliance with the medical staff bylaws, including quality, credentialing, peer review and other functions required by regulatory bodies.

Acts as a liaison and represents medical staff interests to the executive leadership team and the Board.

Physician Staffing and Contracts

Leads provider recruitment and assures full 24 / 7 / 365 provider staffing and on-call coverage

Develops new models of care to assure provider staffing meets the needs of SFCJL patients and residents

Collaborates with medical education leaders on training and mentoring opportunities to assure a pipeline of providers for the future

Regularly reviews and updates SFCJLs physician contracts. Negotiates contracts in collaboration with CFO and CEO. Develops performance metrics and performance evaluation tools to assure delivery of highest quality care.

Assesses provider satisfaction and implements strategies for improvement, as needed.

Assures staffing for pre-employment health examinations

Quality, Safety, Risk and Regulatory functions

Assures timely, high quality, evidence-based clinical care across all SFCJL service lines.

Assures medical staff compliance with all local, state and federal regulations pertaining to their practice. Partners in policy development, deployment and implementation.

In partnership with DON and LNHA, leads quality and safety improvement efforts, using data analysis and trending, such that highest levels of outcomes are achieved.

Engages providers to assure exceptional patient / resident experience.

Conducts case review, participates in root cause analyses and summarizes findings for Quality committee.

Participates in Utilization Management program including attendance at weekly PDPM meetings and consultation on patients / residents with complex issues.

Reviews utilization metrics and partners with clinical and operational leaders on improvement work.

Collaborates with JHRC multidisciplinary team to assure appropriate length of stay and safe transition planning.

Monitors access to outside clinical services and engages appropriate leaders to facilitate timely referrals and care.

Works with IT to assure EHR and clinical systems support clinical needs and the needs of the medical staff.

Committee leadership and representation - Leads or participates on relevant committees including, but not limited to, Medical Executive Committee, QAPI committee, Board Quality Committee, Medical Staff Meetings, Infection Control Committee, Operations Directors Meeting, SFCJL Board of Trustees Meetings

Outreach and Strategic Planning

Assures strong relationships with referring hospitals and other referral sources, provides education and feedback as needed to maintain relationships and to assure safe transitions of care.

Participates in campus-wide strategic planning efforts and assumes a leadership role in the identification, development, implementation, and evaluation of clinical programs and growth opportunities.

Identifies key market trends in elder care, proposes and helps to implement new clinical programs, service lines or areas of excellence to meet future needs

Identifies key market trends affecting physicians in skilled nursing and senior care and develops recommendations to address those trends.

Other duties as assigned.

LICENSE AND CERTIFICATION :

Valid California Medical License (MD or DO).

Valid, unrestricted, Drug Enforcement Agency registration

Board certified in a medical specialty consistent with the type of care provided in the skilled nursing facility, including geriatrics, internal medicine, or physical medicine and rehabilitation.

Member of the California Association of Long-Term Care Medicine, preferred

Certification as a Medical Director by the American Board of Post-Acute and Long-Term Care Medicine or commitment to achieve certification.

EXPERIENCE :

Respected clinician with minimum of five years post-graduate practice in primary care, geriatrics, hospital medicine, or other relevant specialty.

Practice in the skilled nursing environment preferred.

Masters-level training in an applicable field such as public health, business administration, or equivalent preferred.

At least five (5) years of progressively responsible medical management and leadership experience, preferably with medical director experience in a complex skilled nursing environment and / or in an assisted living facility.

Salary based on the balance of administrative and clinical time is negotiable, with administrative time minimum of 80%. Salary will be based on administrative time.
